The OAC is dedicated to ensuring that individuals with obesity are able to seek care by having access to qualified health professionals to help them manage their weight and health.

In order to be listed, you must first complete the form below. Once this form is received, please be advised that it will take about 4 weeks to become officially listed in the database.

Have Multiple Practices? If you have multiple practices and would like to include all of them in ObesityCareProviders.com, you can do so by completing a new form for each location. Please note that you MUST use a different email address for each practice location.

Please Note: The OAC maintains a zero tolerance policy regarding the submission of fraudulent information to ObesityCareProviders.com. Please be advised that all information coming through ObesityCareProviders.com is trackable, and in the event it is suspected or detected that fraudulent information has been submitted, the OAC will work with all necessary law enforcement authorities and provide relevant identification information for further investigation.
